Option 2: Game Day.  Check-In 30 minutes PRIOR TO kickoff of first game.

• Case Community Parks Headquarters 
• Located near the Concession & Restroom

CHECK-IN DOCUMENTS REQUIRED: 

• TEAM ROSTER
  • Player Name, First & Last
  • Date of Birth of Each Player on roster 
  • Jersey # of each player on roster 
• MEDICAL RELEASE FORMS 
  • Every player is required to have a medical release form signed by a parent/guardian 
  • Medical Release Forms MUST be kept with the coach and/or team manager field side 
  • Note that the Medical Release forms do NOT require notarization 
• 9v9 TEAMS ONLY - PLAYER PASS CARDS 
  • Player Pass Card with player name, date of birth, team and club, and association info

Every team must be checked in prior to their first game beginning.  Teams who have not completed check in may have results removed from tournament standings.
