PARENT DOCUMENT UPLOAD
The following 3 documents are required to be uploaded before our registrars can access the player data to add the player to an official game roster.

• Player Photo
• Proof of Birth (Birth Cert, Driver's License, Passport) 
• US Club Medical Release Form - Download English Version; Download Spanish Version 

STEP BY STEP PROCESS 

1. Log in to TGS Parent Account
2. Go to MY CHILDREN
3. Go to the Child's Name

UPLOAD PHOTO
4. Go to EDIT BASIC INFO (under the player name, under TGS ID, under location for photo)
5. Go to CHANGE PHOTO and click on "Drop File Here to Upload"
NOTE: The photo size should be NO BIGGER THAN 200X200 PIXELS.

UPLOAD PROOF OF BIRTH

6. Go to "Billing and Payment Info"
7. Go to Current Program: "WSA 2023-2024 Membership Fee" (or relevant program) 
8. Click on "View Billing & Payment Info"
9. Scroll Down to Agreements & Forms
10. Select Payment Plan Option. 
11. If MISSING is reported by Birth Certificate/Passport then click on UPLOAD
12. Choose the Proof of Birth File (Proof of Birth Options: Birth Cert, Passport, or Driver's License)

UPLOAD US CLUB MEDICAL RELEASE & LIABILITY FORM

13. If MISSING is reported on US CLUB FORM then download the form and complete form.
14. Download US CLUB FORM HERE IF NEEDED - US CLUB FORM ENGLISH.   US CLUB FORM SPANISH.
15. Click on UPLOAD next to Missing US Club Form and Select the File.
NOTE: Very important:  You have to upload this document next to US CLUB FORM for it register with your child's compliance per US Club Soccer.
16. Success!  You have completed the needed player document uploads. 👏
