THE REGISTRATION COMMITMENT FORM
PLAYER REGISTRATION COMMITMENT PAPER COPY FOR FIELD USE: This document can be utilized at "the fields" to have players "commit" to your team. WHY? Sometimes you will have players leave the tryout location after the session, and then not complete registration for days or weeks. This leaves you in limbo on whether the player has actually "committed". This document creates a written agreement between PARENT/PLAYER & CLUB. It is important that the parent sign this document for it to be contractually valid. Use the "Team Roster #" box in the upper right to keep tabs on the "number of players" you have offered. You will keep the REGISTRATION COMMITMENT documents on your person.
PLAYER REGISTRATION ONLINE COMMITMENT: This online REGISTRATION COMMITMENT is a backup in case a player leaves the field without completing the paper copy. The player may do this on accident, or you forget, or she/he is undecided. This online link can be shared and the player can commit via this online process in less than 60 seconds.
PLAYER CHECK-IN FORM - DOWNLOAD & PRINT: This is a form that can used to "check in" players at tryouts. It also includes a "team organizational" form that is handy if you are hosting an age group session and forming multiple teams.
